Medical Treatment for Varicose Veins and varicosities

Treatment Measures:

- Recommended:

* Sports like running, cycling and swimming.

* Rest at night with a slight elevation of the legs by shims 10 to 15 cm in height under the foot of the bed.

* Finish the bathrooms and showers with cold water.

* Specific gymnastics.

* External containment by elastic bandages and medical stockings, suitable for each patient.

* Massage by qualified personnel.

* Self-massage ...

- Avoided:

* Prolonged exposure to sunlight.

* The use of flat shoes or high heels.

* Orthostatism prolonged low mobility.

* Clothing too tight.

* Sedentary lifestyle.

* Overweight.

- Control of risk factors: obesity, multiple pregnancies, heredity, age, etc.

The elastic restraint:

It is very important to bring the containment limb circumference to sell, and use elastic bands with good extensibility, which can be adapted to situations of rest and muscular activity. In the foot and leg are used from 8 to 1O cm wide respectively and for those of 1O cm thigh and especially those of 12 cm.

The mechanism of action:

-Decrease in vessel diameter, with a significant reduction in reflux and the venous pressure.

- Improving the effectiveness of the "muscle pump" of the calf.

- Intratisular increased pressure, which contributes to an easier absorption of edema.

- In sclerosing treatment of varicose veins.

- After varicose vein surgery.

- In certain phlebostatic ulcers.

- Edema.

The elastic bands are often prescribed temporarily, for short periods of time. For long periods are often prescribed therapeutic stockings. Placing the elastic bandage should be initiated at the base of the fingers to wrap around the foot to go to reach the desired level, leg or thigh, with a decreasing compression.

Contraindications of elastic containment: arterial obliterans of lower limbs, microangiopathy evolved decompensated heart failure, infections and abscesses of the lower limbs.

Venous tonic

Acting on smooth muscle of the vein wall. We can increase venous tone by stimulating alpha-adrenergic receptors or inhibiting degradation of norepinephrine.

- Ergot derivatives: dihydroergotamine and dihidroergocristina.

The ergot is a fungus (Claviceps purpurea) parasite of rye. Exerts a vasoconstrictive action on muscle fibers, both arterial and venous network. It also increases the force of contraction of "muscle-venous pump" of the calf. Contraindicated at the beginning and end of pregnancy.

- Ruscus aculeatus: increased sympathetic tone and thus the venous tone.

- Horse Chestnut (Aescillus hippocastanum): Reduce the size of varicose veins.

- Routine (Route chalepensis, Ruta graveolens): From its leaves or is removed rutoside rutina.Incrementa capilares.Produce resistance of an increase in venous tone, and an improvement of symptoms.

Reservoir function

While increases venous tone, decrease capillary permeability and increase their resistance.

- Soluble Bioflavonoids: capillary permeability decreased by inhibition of hyaluronidase, representing an increase in the resistance of the vascular wall.

* Among the natives, are the vitamins P.

* Among recent semisynthetic preparations highlight the "diosmin" and "hidrosmin."

# Diosmin: decrease capillary permeability, while inhibiting the degradation of norepinephrine. It also diminishes the capillary permeability. It can be delivered during pregnancy.

# Hidrosmin: venotropa has an ERA higher than diosmin. Improving the partial pressure of oxygen in the tissues.

# Anthocyanosides Myrtyllus Vaccinium (blueberry): blue-black berries are rich in anthocyanins factors of vitamin P properties, behaving as capilatropos, anti-edema and anti-inflammatory.

# The escin (Aescin) is poorly absorbed from the gastrointestinal tract and peak does not appear until 16 hours absorption, eliminated by the kidneys and bladder. Also absorbed percutaneously. Antiedematosa effect, also increasing the tone of the vein wall.

# Dobesilate calcium: synthetic substance that has a vein myotonic activity and, moreover, improves microvascular resistance, decreasing capillary permeability and improve lymphatic drainage.

Involved in the control of the extravascular substances through the interstitial fluid absorption.

- Melilotus officinalis: very rich in vitamin C. This clover is removed and a coumarin derivative thereof, the 5-6 alpha-benzopyrene. Coumarin induces proteolysis of high molecular weight protein, oncotic pressure down and reduce the edema, but does not alter blood clotting. Its half-life is short and is excreted by the kidneys. Melilotus extracts contain flavones with vitamin P properties' also act as reducing capillary permeability and causing an improvement in the strength of them.

- Diuretics quickly mobilize interstitial fluid, thereby reducing edema. In varicose disease have been more effective thiazide diuretics. Should be used only in those patients in whom edema is significant, and always with due caution about the possible systemic effects and side effects.
